If you have ‘lymphedema’ or swelling in your arms or legs everyday activities can be frustrating.  Swelling occurs when there is an accumulation of ‘lymph’ fluid in the soft tissue between your skin and muscles.  As swelling worsens pain, skin changes, water blisters, weeping of fluid, infection, mobility problems and disability can develop.

In simple terms, if you have leg or arm swelling increased lymph fluid creation overloads your normal lymphatic system or a defective lymph system is unable to remove the normal amount of lymphatic fluid. 

There are two types of lymphedema. Primary lymphedema is an inherited problem of the lymphatic system (onset from birth to adulthood). Secondary lymphedema is acquired with multiple causes including complications of cancer surgery and therapy in the US; vein disease, thyroid, heart, liver, kidney disease; and medications.

How Does Lymphedema Therapy Help?

The goal of lymphedema therapy is to reduce the burden of living with chronic swelling and empowering each patient to overcome the physical and emotional suffering of lymphedema. This happens when you learn the techniques to control the swelling, restore the functional capacity of the patient and prevent infection.

Tenets of Lymphedema Therapy?

The mainstay of Lymphedema Therapy is Manual Lymph Drainage  (MLD)

  1. Reroutes lymphatic flow via alternatives pathways with a specialized massage technique for improved lymph flow. 
  2. Limitations to MLD: a skin infection, blood clots, or active disease in the lymph drainage area.


Improving Lymphedema with compression modalities: (one or more modalities: used according to the type and degree of swelling.)

  1. Multilayer lymphedema wraps.
  2. Graduated compression garments/socks: compression gradually decreases as you move up the extremity.
  3. Pneumatic compression sleeves: worn on the affected limb up to an hour per day applying intermittent compression. 
  4. Medical grade graduated compression: tighter at the hands and feet gradually decreases up the extremity. 


Exercise: 

  1. Encourages lymph flow and helps clear swelling.
  2. Maintain and improves your strength and muscular pump.  
  3. Wearing compression while exercising helps clear swelling.
  4. Targeted exercise helps manage swelling and pain.


Skincare is important: 

  1. Improves and maintains skin integrity 
  2. Decreases the risk of bacterial and fungal infection

What is Lymphedema Therapy Like?

Lymphedema therapy is an ongoing regimen of care. Occupational therapists and physical therapists, along with OT Assistants and PT Assistants with additional training, experience, and credentials to provide lymphedema care.  

Your therapy plan will depend on: 

  1. Which body area is affected.
  2. The underlying cause of your lymphedema 
  3. Your unique needs as a patient.
